1. Who we are & acceptance of these Terms

These Terms of Service ("Terms") are a binding agreement between you and [LEGAL ENTITY NAME] ("ShadedMD," "we," "us"), located at [BUSINESS ADDRESS]. By accessing our website or purchasing an evaluation, you agree to these Terms, our Privacy Policy, and the Telehealth Consent in Section 4. If you do not agree, do not use the service.

2. What the service is (and is not)

ShadedMD is a technology platform that connects you with independent, U.S.-licensed physicians who conduct asynchronous telehealth evaluations for medical window tint exemptions. We are not a medical group, pharmacy, or insurer, and we do not practice medicine. All medical decisions are made solely by the reviewing physician exercising independent professional judgment.

No guarantee of approval. Submitting a questionnaire and payment does not guarantee that a physician will approve an exemption. If your evaluation is not approved, you receive a full refund as described in Section 6.

Not for emergencies. The service is not for urgent or emergency medical needs. If you are experiencing a medical emergency, call 911.

3. Eligibility

You must be at least 18 years old, located in the United States, and the person about whom the medical information is submitted (or that person's legal guardian). You agree to provide accurate, current, and complete information; certificates issued based on false information are void.

4. Telehealth Consent

By submitting the questionnaire you consent to receive care via asynchronous telehealth, meaning a licensed physician will review the information you provide without a real-time video or in-person visit. You understand that: (a) telehealth has limitations compared to in-person care, and the physician may decline to issue a certificate and instead recommend in-person evaluation; (b) you may be contacted for clarifying information; (c) you may request a copy of your records or withdraw consent at any time by contacting support@shadedmd.com; and (d) the reviewing physician is licensed in the state you select.

5. Fees & payment

The fee for an evaluation is shown at checkout (currently $129, one-time). Payment is processed by our third-party payment processor; your full card number never touches our servers. Prices may change, but never after you have paid.

6. 30-day money-back guarantee & refunds

If the reviewing physician does not approve your exemption, we automatically refund your full purchase price to your original payment method. For any other reason, you may request a full refund within 30 days of purchase by contacting support@shadedmd.com. Refunds are processed within 5–10 business days.

7. Your certificate & legal compliance

Your certificate documents a physician's medical recommendation. You are responsible for complying with your state's law, including filing any required DMV forms, carrying the certificate in your vehicle, renewing on time, and keeping your tint within any limits that still apply to exempt drivers in your state. Tint laws change; information on this site is provided for convenience and is not legal advice.

8. Prohibited use

You may not submit information about another person without authorization, alter or resell certificates, use the service to evade law enforcement, or scrape, copy, or reverse-engineer the platform.

9. Intellectual property

The site, its design, text, graphics, and software are owned by [LEGAL ENTITY NAME] and protected by law. We grant you a limited, personal, non-transferable license to use the site for its intended purpose.

10. Disclaimers & limitation of liability

THE SERVICE IS PROVIDED "AS IS" WITHOUT WARRANTIES OF ANY KIND. TO THE MAXIMUM EXTENT PERMITTED BY LAW, OUR TOTAL LIABILITY FOR ANY CLAIM ARISING FROM THE SERVICE IS LIMITED TO THE AMOUNT YOU PAID US IN THE 12 MONTHS BEFORE THE CLAIM. WE ARE NOT LIABLE FOR INDIRECT, INCIDENTAL, OR CONSEQUENTIAL DAMAGES, OR FOR CITATIONS, FINES, OR VEHICLE-INSPECTION OUTCOMES.

11. Dispute resolution
